A 78-year-old woman was referred for reduced vision in her left eye. Fundus imaging (a) showed a a choroidal macrovessel extending temporally (arrow), a perifoveal circumscribed lesion (arrowhead) and a patchy atrophy (asterisk). Indocyanine green angiography (b) revealed early filling, and hyperfluorescence surrounded by a hypofluorescent halo caused by a choroidal neovascular membrane (arrow) extending from the choroidal macrovessel. Fluorescein angiography (c) demonstrated late leakage from the choroidal neovessel (CNV) (arrowhead) and hyperfluorescence nasal to it due to the patchy atrophy (arrow). OCT (d) showed a pigment epithelial detachment (arrow), subretinal hyperreflective material (asterisk), and serous retinal detachment (SRD) overlying the macrovessel. The OCT one month following a single intravitreal anti-VEGF injection (e) demonstrated a complete SRD resolution while the OCT-A individualized the CNV.
