A 37-year old man, with surgically corrected congeni- tal heart disease (repaired tetralogy of Fallot [rTOF] and subsequent biological pulmonary valve replacement with patch augmentation to pulmonary trunk and right ventricu- lar outflow tract [RVOT]), was referred to our institution to undergo ventricular tachycardia (VT) ablation for recurrent syncopal VT episodes. The majority of VTs in rTOF are related to right ventricular (RV) anatomically defined isth- muses [1, 2]; however, VT ablation in these patients might be difficult due to complex anatomy and to unmappable VTs.
