Extracranial carotid artery aneurysm (ECAA) is a rare occurrence. Despite the increasing role of endovascular procedures, open repair seems to be the gold standard for ECCAbased on the patient's anatomy and comorbidities, and on the physician's preference. We report the case of an ECAAsurgically treated and we reviewed the different therapeutic experiences in the literature.
