Objective: the objective of this article is to describe the possible causative agents of Aortic IntraMural Hematoma (IMH) Design: we have selected an autoptic report of cardiac death that showed a significant IMH. Case: a 45-year-old man with hypertension, left ventricular hypertrophy and coronary artery disease. Results: coronary artery disease may have played a significant role because in the ascending aorta vasa originate from coronaries; the left ventricular hypertrophy suggests that the systolic pressure stress of aorta may have caused an internal compression action; the resuscitation operations may have determinate a traumatic external pression on ascending aorta. Conclusion: it's no possible exclude the different pathogenic mechanisms of IMH with autoptic findings.
