After a brief illustration of the principal clinical aspects of the neovascular glaucoma and a re-examination of the treatments (medicai, surgical, etc.), the Authors report their experience in the treatment of the neovascular glaucoma using the Nd: Yag laser. 24 patients wiith neovascular glaucoma connected with diabetic retinopathy and vascular occlusion in lifeless and aching eyes, were treated. The average preoperative intraocular pressure was 40 mmHg. The transcleral cyclophotocoagulation was realized with a Lasag Microruptor II Nd: Yag laser in the thermal mode (free running], alter topical and retrobulbar anesthesia. Thus, it was able to reach, through the sclera, the ciliary body in order to destroy the ciliary epithelium and the associated vessels and therefore to reduce the production 0f the aqueous humor. The successful results in the reduction 0f the IOP and of the pain have shown this uninvasive and ambulatory techniques as an effecttive alternative to traditional
Dopo aver brevemente illustrato i principali aspetti clinici del glaucoma neovascolare e riesaminato i numerosi tipi di trattamento utilizzati (medico, chirurgico e parachirurgico), gli AA. riportano la propria esperienza nel trattamento del glaucoma neovascolare con Nd: Yag laser. Sono stati trattati 24 pazienti con glaucoma neovascolare secondario a diabete e ad occlusione vascolare in occhi funzionalmente spenti e dolenti. La pressione oculare media prima del trattamento era di 40 mmHg. Dopo anestesia topica e retrobulbare la ciclofotocoagulazione transclerale è stata eseguita con apparecchio Lasag Microruptor II Nd: Yag laser in modo termico (free running) per poter raggiungere attraverso la sclera il corpo ciliare, provocandone la distruzione e quindi riducendone la produzione di umor acqueo. I favorevoli risultati nella riduzione della PO e del dolore hanno dimostrato che questa tecnica non invasiva e ambulatoriale si può proporre come efficace alternativa alle terapie tradizionali.
