Cardiac rupture is a rare complication of myocardial infarction that most commonly affects the anterior or lateral wall and the main risk factors are advanced age, female sex and coronary obstruction. In this paper the authors present an autopsy report of death by cardiac rupture in 50-years-old man with apical-posterior side and coronary artery disease (CAD) without obstruction. The clot positioned in ventricular site of rupture does not support with any certainty if the death event has extended over time because it is not excluded that the clot may have formed during the immediate peri-mortem or post-mortem.
