The aim of surgery in Peyronie's Disease is to guarantee a penis that is "functionally straight" and with enough rigidity to allow the patient to resume penetrative sexual intercourse. The type of surgery should be tailored to the degree and type of deformity, and on the quality of erections at the time of the preoperative assessment. Plication procedures are indicated in patients with adequate erections, with or without drug assistance, to correct simple curvatures of less of 60°, when the presumed loss of length caused by the plication will be less than 20 % of the total preoperative erect size. These techniques are usually not indicated in patients with complex curvatures and hourglass deformities, as they would lead to a poor cosmetic and functional outcome due to excessive shortening or the persistence of hourglass indentation. © 2013 Springer Science+Business Media, LLC.
