INNOCENZI Porokeratosis palmaris et plantaris disseminata is a rare autosomal dominant chronic disorder of keratinization characterized by annular plaques with an elevated hyperkeratotic border on the palms and soles and small round hyperpigmented macules surrounded by a hyperkeratotic rim in legs, arms and forearms. Biopsy showed the istological features of cornoid lamella caracterized by a column of parakeratosis overlying an epidermal depression with dyskeratotic keratinocytes. We report a case of PPPD in a 65 years old man and in his son of 35 years. Both patients presented lesions on the palms and soles, on the trunk and on the legs. From the clinical appearence and histological findings a diagnosis of porokeratosis plantaris palmaris et disseminatis was made. The patients treated with acitretin (0,5 mg/kg/die) showed a clinical improvement after four months of treatment.
