Cervical carcinoma is the second most common malignant disease worldwide, often occurring in young women who have not completed childbearing. In this era of emphasis on conservative treatments, management of cervical cancer could be influenced by patient desire to preserve fertility, maintaining the uterine corpus. Herein is reported the case of a young woman with locally advanced cervical cancer that was successfully treated using neoadjuvant chemotherapy followed by simple vaginal trachelectomy and laparoscopic lymphadenectomy. The success of such an approach, which is not the standard of care in this disease, suggests that additional studies should be performed in a selected population. © 2011 AAGL.
