In this study we have sought possible differences in the initial and short-term hypotensive effects of nifedipine in hypertensive patients in relation to their renin profile. A highly significant decrease in BP was observed in low-renin and normal-renin hypertensive patients, while a less pronounced fall was found in high-renin hypertensives. Furthermore, 55% of low-renin hypertensives had their diastolic BP normalized after 1 week of therapy, whereas the same result was obtained only in 33% of high-renin hypertensives. These data indicate that nifedipine can be used as first-choice drug in low-renin hypertension, although pre-treatment renin profile might not be employed as a sure predicting index of a short-term efficacy of this drug.
