IMMUNOSUPPRESSIVE drugs that are currently in use effectively inhibit the T-lymphocyte–dependent immune response. The introduction of cyclosporine (CyA) in the early 1980s marked a major improvement in the shortterm results following renal transplantation. By the end of that decade a new IL-2 (interleukin-2) inhibitor, tacrolimus, became available in the clinic. Twenty years have passed since the introduction of the first of these types of drugs, and the down-regulation of IL-2 via calcineurin inhibition continues to be the cornerstone of immunosuppressive therapy following solid-organ transplantation. Other immunosuppressive drugs, including those that have become available recently, such as mycophenolate mofetil (MMF), sirolimus, and others, play an important role, not as cornerstone regimens, but as adjunctive therapies.

Abstract
