carotid endoarterectomy (CEA) represents surgical repair for symptomatic high-grade carotid artery stenosis, several trials showed that the risk of stroke is significally reduced by endoarterectomy compared with medical therapy alone. Carotid artery angioplasty and stenting (CAS) has been proposed as an alternative to surgery. The purpose of our study was to determine patients that were suitable for each type of intervention and to analyse the benefits and the risk of the procedure. The study compared CAS to CEA in 76 pz with potentially increased procedural risk. The main result was that protected stenting does not appear to be inferior to endoarterectomy as revascularization strategy. Despite the use of cerebral protection devices the neurologic complication rate in stenting group was hogher than CEA group.
