Purpose: To evaluate the efficacy of photorefractive keratectomy (PRK) in the treatment of hyperopic esotropia in young adults. Methods: We studied thirty patients with a mean age of 30.8 years. Subjective and objective methods were used to evaluate hyperopia. Uncorrected and corrected esotropia for far and near vision was quantified by cover tests and prismatic lens correction. All patients were subjected to PRK treatment. Results: Pre–operative best spectacle corrected visual acuity (BSCVA) was 20/30. There was a decrease in BSCVA in the first month after treatment. However, BSCVA returned to preoperative values at two months. One year after PRK treatment the uncorrected visual acuity (UCVA) was improved in 22 patients and stationary in 4 patients. In 4 patients there was a myopic shift in the first month. At the end of the follow up period all patients were ortophoric with no correction. Conclusions: Our results demonstrate that photorefractive keratectomy (PRK) reduces accommodative stress and excess of convergence with consequent ocular deviation. It can be a valid approach to the management of hyperopic esotropia by relieving spectacle dependence and reducing accommodation.

Scopo: Valutare l'efficacia della cheratectomia fotorefrattiva (PRK) nel trattamento dell'esotropia ipermetropica nei giovani adulti. Metodi: Abbiamo studiato trenta pazienti con un'età media di 30,8 anni. Per valutare l' ipermetropia sono stati utilizzati metodi soggettivi e oggettivi. L'esotropia non corretta e corretta per la visione da lontano e da vicino è stata quantificata mediante test di copertura e correzione della lente prismatica. Tutti i pazienti sono stati sottoposti a trattamento PRK. Risultati: : L'acuità visiva corretta pre-operatoria (BSCVA) era 20/30. C'è stata una diminuzione della BSCVA nel primo mese dopo il trattamento. Tuttavia, BSCVA è tornato ai valori preoperatori a due mesi. Un anno dopo il trattamento con PRK, l' acuità visiva non corretta (UCVA) è stata migliorata in 22 pazienti e stazionaria in 4 pazienti. In 4 pazienti si è verificato un cambiamento miopico nel primo mese. Alla fine del periodo di follow-up tutti i pazienti erano ortoforici senza correzione. Conclusioni: I nostri risultati dimostrano che la cheratectomia fotorefrattiva (PRK) riduce lo stress accomodativo e l'eccesso di convergenza con conseguente deviazione oculare. Può essere un valido approccio alla gestione dell'esotropia ipermetropica alleviando la dipendenza dallo spettacolo e riducendo l'accomodazione.
