The theranostic potential of 177Lu-PSMA-617 radioligand therapy, used for the management of metastatic Castration Resistant Prostate Cancer (mCRPC), arises from the decay scheme of 177Lu, characterized by therapeutic β− with associated γ emissions, suitable for post-administration imaging. This enables a personalized treatment optimization strategy based on the assessment of radiopharmaceutical activity distribution and patient specific biokinetic washout through lightweight wearable γ-ray detectors positioned on the patient’s body for continuous monitoring over time. Here, preliminary results on the expected performance of these devices under realistic clinical scenarios evaluated by means of FLUKA Monte Carlo simulations are presented.
