We report a posterior capsular rupture (PCR) case in a patient who underwent cataract surgery for a posterior polar cataract. This case is noteworthy as the PCR occurred during stromal hydration of the corneal incisions, a phase typically considered low risk. This work highlights the corneal hydration maneuver as a potentially critical moment for PCR.
