GLP-1 RAs have evolved from glucose-lowering agents into potent, system-wide modulators of vascular, metabolic, and neurobehavioral pathways, but this same pleiotropy complicates attribution of benefit and risk at the individual patient level. As doses escalate, mechanisms multiply, and background cardioprotective therapy intensifies, the marginal gain in hard outcomes must be weighed against cumulative gastrointestinal, biliary, autonomic, neuropsychiatric, and economic burdens that may not be trivial over years of exposure.
