Technique de triple neurectomie par rétropéritonéoscopie pour douleurs neuropathiques inguinales chroniques postopératoires (avec vidéo)

This video shows the case of a 44-year-old man who underwent laparoscopic surgery for a right inguinal hernia (total extra-peritoneal parietoplasty [TEPP]). He developed chronic postoperative inguinal pain, and underwent two laparoscopic reoperations (removal of a prosthesis fragment; adhesiolysis of the anterior parietal peritoneum). Because of the persistence of the pain syndrome with a neuropathic component in the genitofemoral and right ilio-inguinal territories, he was treated in pain medicine. The pain was refractory but transiently improved by test blocks. Consequently, the indication of triple inguinal neurectomy was proposed. Pain territory mapping was carried out the day before surgery and matched the territories described.
