Introduction: Liver cirrhosis from alcohol abuse represents one of the main indications for liver transplantation. Since there is difference among alcohol abuser, according to DSM- 5 criteria, a careful psychological evaluation to assess the risks of relapse and the effective abstinence are the need before liver trans- plantation. Aim and Methods: From January 2019 until June 2020, among 212 patients who underwent evaluation for OLT, 46 patients had past history of alcohol consumption. Of these patients 26 were real abuser; 20 showed casual consumption, without effective addic- tion. Among these 46 patients, 16 underwent liver transplantation. Pre-transplant psycodiagnostic evalutation by tests (SCL.90m, BDI- II, BAI and IPQ-r), individual and family member interview were performed. Results: Pre-OLT evaluation tests showed the following signifi- cant data: SCL: 90-R: somatization (61.73), depressive symptoms (58.95), anxiety (59.33); BDI II: somatic-affective factor (moder- ate level); BAI: anxiety (moderate level). Post-transplant retesting showed non-significant data with any patient showing relapse in alcohol consumption after OLT. Conclusions: Our data show that among patients listed for alco- hol consumption, 26 were alcohol addicted, according to DSM-5 criteria and 20 had casual comsumption without real addiction. The pre and post –transplant psychological workout and support allowed relapse in alcohol consumption. Patients without a real ad- diction were supported in trying to change daily habits and favour abstinence. Patients with clear addiction needed a structured psy- chotherapy.
