Cefiderocol (FDC), a siderophore-cephalosporin con- K. pneumoniae carbapenemase (KPC) variants, in jugate, is the newest option for treating infection with combination with a conjugative low copy number vector carbapenem-resistant gram-negative bacteria. We carrying the fec gene cluster from pKPN. We observed identified a novel mechanism contributing to decreased transcriptional repression of fiu, cirA, fepA, and fhuA FDC susceptibility in Klebsiella pneumoniae clinical siderophore receptor genes in blaKPC–fec–E. coli cells isolates. The mechanism involves 2 coresident plas- treated with ferric citrate. Screening of 27,793 K. pneumids: pKpQIL, carrying variants of blaKPC carbapen- moniae whole-genome sequences revealed that the emase gene, and pKPN, carrying the ferric citrate fec cluster occurs frequently in some globally distrib-transport (FEC) system. We observed increasing FDC uted different KPC–producing K. pneumoniae clones MICs in an Escherichia coli model system carrying (sequence types 258, 14, 45, and 512), contributing to different natural pKpQIL plasmids, encoding different reduced FDC susceptibility.
