We examined on a 61-year-old man who presented with a persistent nodule on his right leg, recently causing itching and concern. Despite anamnestic, clinical, and dermoscopic data initially suggesting a benign condition, such as dermatofibroma, further investigation was prompted by the atypical coloration and the absence of the characteristic dimple sign. Histopathological examination and immunohistochemistry confirmed the diagnosis of Primary Cutaneous Cribriform Apocrine Carcinoma (PCCAP), an exceptionally rare neoplasia. This manuscript critically analyzes and discusses the deceptive dermoscopic features of PCCAP, aiming to enhance awareness among dermatologists about this infrequently encountered and not fully understood malignancy.

Abstract
