The novel severe acute respiratory syndrome coronavirus (SARS-CoV-2) and the associated coronavirus disease 2019 (COVID-19) continue to spread globally, with over 265 million confirmed cases. In Italy, the main strategy to prevent virus transmission has been focused on unvaccinated individuals. However, as of December 13, 2021, 19,212 COVID-19 positive cases were reported, indicating that vaccinated individuals also play a significant role in virus transmission. From June to November 2021, the Altamedica Laboratory detected 866 COVID-19 cases using real-time RT-PCR analysis. Among these cases, 623 (72%) were fully vaccinated with BioNTech/Pfizer BNT162b2, Moderna, or AstraZeneca vaccines, and 243 (28%) were unvaccinated. Of the vaccinated cases, 31% were asymptomatic, 69% symptomatic, with 7% hospitalized and 1% deceased. Among the unvaccinated cases, 67% were symptomatic, with 12.7% hospitalized and 2% deceased. These findings align with national and international data, emphasizing that vaccinated individuals remain a significant part of the pandemic. The study suggests that relying solely on vaccination, especially nontraditional vaccines targeting only the spike protein, is insufficient to achieve lasting protection, underscoring the importance of herd immunity
