This work presents a brief overview of the main steps required for post-incident analysis, outlines some procedures which enable to effectively investigate the causes of accidents in process plants and shows how they may be applied to real cases. In fact, a better understanding of past incidents can be obtained also applying only some steps of the procedure, based on the available information. The applications refer to three major accidents occurred in Italy, at an oil products storage facility in Naples (1985), at a maleic anhydride plant in Colleferro (1981) and at an oil refinery in Milazzo (1993), respectively. Different procedures are applied to these study cases according to the detail of the available information.
