Here we present an intriguing clinical case of a pancreatic NEC (pNEC) histologically characterized by a poor differentiation grade and a severe fibrosis, but a relatively low Ki67 LI, that showed discordance between the aggressive morphology and the NET-like radiotracer uptake and clinical behavior.
