Melanonychia is a diagnostic challenge for clinicians. The most serious disease of the nail unit is subungual melanoma (SM), which primarily shows longitudinal melanonychia (LM). However, only 6.8% cases of LM show histologic evidence of SM. Other diseases that could mimic SM include subungual hematoma, nail matrix nevus, trauma, nail apparatus lentigo and druginduced LM.
