Abstract: : Purpose: Excimer laser is recently being used for therapeutic scopes in the reduction of corneal opacification and in cases of recurrent corneal erosion. We studied the efficacy of photherapeutic keratectomy (PTK) in the treatment of corneal scars and recurrent corneal erosion. Methods: A total of 17 patients were enrolled in the study. 10 eyes were treated for corneal scars and 7 eyes were treated for recurrent corneal erosions with the excimer laser. The follow-up was 15 months. Results: In the eyes with corneal scars the mean preoperative best corrected visual acuity (BCVA) was 20/60; after treatment, this improved to a mean value of 20/40. Four eyes showed improvement of 2 or more Snellen lines and four improved of one line or showed no change. In one patient with deep scarring a second treatment with combined PRK led to improvement. A complication was reactivation of herpetic infection in one patient treated for a post herpetic corneal scar even though the last episode of keratitis had occurred 2 years earlier. In the patients with recurrent corneal erosion there was a success rate of 86% after one treatment and 100 % after two treatments. There was a subjective improvement in the quality of vision in some patients. Conclusion: PTK offers good results in the treatment of RCE. In corneal scars many factors contribute to the success of treatment including scar aetiology, the depth and the number of scars. Proper patient selection in these cases is mandatory.
: Scopo: Il laser ad eccimeri è stato recentemente utilizzato per scopi terapeutici nella riduzione dell'opacizzazione corneale e nei casi di erosione corneale ricorrente. Abbiamo studiato l'efficacia della cheratectomia fototerapeutica (PTK) nel trattamento delle cicatrici corneali e dell'erosione corneale ricorrente. Metodi: Un totale di 17 pazienti sono stati arruolati nello studio. 10 occhi sono stati trattati per cicatrici corneali e 7 occhi sono stati trattati per erosioni corneali ricorrenti con il laser ad eccimeri . Il follow-up è stato di 15 mesi. Risultati: Negli occhi con cicatrici corneali l'acuità visiva media preoperatoria è stata corretta meglio (BCVA) era 20/60; dopo il trattamento, questo è migliorato fino a un valore medio di 20/40. Quattro occhi hanno mostrato un miglioramento di 2 o più linee di Snellen e quattro sono migliorati di una linea o non hanno mostrato alcun cambiamento. In un paziente con cicatrici profonde un secondo trattamento con PRK combinato ha portato a un miglioramento. Una complicazione è stata la riattivazione dell'infezione erpetica in un paziente trattato per una cicatrice corneale post-erpetica anche se l'ultimo episodio di cheratite si era verificato 2 anni prima. Nei pazienti con erosione corneale ricorrente si è verificata una percentuale di successo dell'86% dopo un trattamento e del 100 % dopo due trattamenti. C'è stato un miglioramento soggettivo nella qualità della vista in alcuni pazienti. Conclusione: offerte PTK buoni risultati nel trattamento della RCE. Nelle cicatrici corneali molti fattori contribuiscono al successo del trattamento, tra cui l'eziologia della cicatrice, la profondità e il numero delle cicatrici. In questi casi è obbligatoria una corretta selezione del paziente .
