Changes of corneal thickness and endothelial ultrastructure of rabbit eyes after radial keratotomy as well as effects of prednisolone and bendazac-lysine on keratectomized corneas were studied. The corneal thickness increased after keratotomy and the maximum values of control eyes were measured on the seventh day postoperatively; they returned to normal after 2 months. Swelling of mitochondria, disappearance of cristae, increased synthetic activity, and elaboration of cell junction were found with electron microscope. We conclude that direct trauma or postoperative inflammation plays a minor role in the pathogenesis of endothelial alterations, while elaboration of free radicals during normal regenerating processes may be a principal cause of endothelial changes. Prednisolone treatment was ineffective in preventing the development of corneal edema and endothelial alterations. Bendazac-lysine treatment decreased both corneal edema and endothelial changes.
