This paper introduces two related measures that can be used to evaluate, describe, and track changes in personality functioning. Both are derived from the Shedler-Westen Assessment Procedure (SWAP-200). The first measure is the Personality Health Index (PHI) that offers an assessment of personality functioning that may be applied to the study of outcome in any treatment purporting to affect overall psychological health, such as long-term psychodynamic therapy and psychoanalysis. Preliminary studies suggest robust construct validity for the PHI. The second measure, RADIO, uses the item content of the SWAP-200 to categorize personality functioning into five domains: Reality testing and thought process, Affect regulation and tolerance, Defensive organization, Identity integration, and Object relations. Referred to by the acronym RADIO, this measure provides a picture of personality health across major dimensions of personality functioning. Applying these measures to the well-studied case of “Mrs. C” illustrates their potential clinical, educational and research utility. Both the PHI, a global measure, and the RADIO, a more specific measure, demonstrate changes in her functioning from early to late periods in her psychoanalysis. The paper elaborates the wider application of the PHI and RADIO in clinical and research contexts.
Il capitolo presenta due misure, tra loro collegate, che possono essere utilizzate per valutare, descrivere e monitorare i cambiamenti nel funzionamento della personalità. Entrambe derivano dalla Shedler-Westen Assessment Procedure (SWAP-200). La prima è il Personality Health Index (PHI), che può essere applicato allo studio dell’esito di qualsiasi trattamento che si proponga di incidere sul benessere psicologico globale, come una terapia psicodinamica a lungo termine o una psicoanalisi. Studi preliminari hanno dimostrato una buona validità di costrutto del PHI. La seconda misura, il RADIO, utilizza il contenuto degli item della SWAP-200 per categorizzare il funzionamento della personalità in cinque ambiti: l’esame di Realtà e i processi di pensiero, la regolazione e la tolleranza degli Affetti, l’organizzazione Difensiva, l’integrazione dell’Identità, e le relazioni Oggettuali. Questa misura, alla quale ci riferiremo con l’acronimo RADIO, fornisce un quadro complessivo degli aspetti sani della personalità considerando le principali dimensioni del suo funzionamento. L’applicazione di queste due misure al ben noto caso della “Signora C.” illustra la loro potenziale utilità nell’attività clinica, educativa e di ricerca. Sia il PHI, una misura globale, sia il RADIO, una misura più specifica, permettono di rilevare i cambiamenti nel funzionamento della personalità della paziente dalle fasi iniziali a quelle finali della sua psicoanalisi. Questo studio illustra quindi come il PHI e il RADIO possano essere applicati nei contesti clinici e di ricerca.
