"help, or at least do no harm.” Hippocrates’ words “Chance favors the prepared mind” Louis Pasteur “There is a medicine for every disease except ageing, who has the knowledge knows it, and who fails to know, can’t” The Prophet Mohammed, P.B.U.H Library shelves contain many books on Sexual Medicine, but nearly none is devoted solely to Drug discovery issues. Many of the books that do discuss the subject of Sexual Medicine relate to its scientific basis or to some specific clinical aspects rather than to basic science and Drug discovery issues. The specialty of Sexual Medicine continues to grow and change rapidly. There have been major areas of advances in just the last decade. This book reaches us in a moment when our scientific community has received visible, unprecedented attention to sexual dysfunction, thanks to the new (expensive) diagnostic tools of molecular biology, genetics, pharmacology, biochemistry, physiology and so forth. This book provides comprehensive coverage of both basic and the most advanced approaches to the drug discovery issues in the field. We are on the verge of an era of translation of bench side discoveries to clinical therapies. We hope that this book will enlighten all of these areas, and supply guidance where it is needed most. The book attempts to provide a balanced view of the drug discovery issues and the number of experts from the academic departments and industry sharing their knowledge and experience with the readers gives the book an outstanding value. All contributors provide with each chapter an up-to-date review on key topics in Sexual Medicine. Hopefully, some of these concepts will find clinical application in the following years. There are twenty chapters: Chapter 1 provides a snapshot about the advances in the mechanism of erection with special focus on therapeutic targets. Chapter 2 describes the advances in the mechanism of ejaculation. Chapter 3 details the advances in pathophysiology of erectile dysfunction Chapter 4 explains the lessons learned from knockout mice and their possible applications in the field of sexual medicine. Chapter 5 summarizes the progress and problems in exploration of therapeutic targets and describes the process of drug discovery. Chapter 6 details the future of targeting smooth muscle for sexual disorders. Chapters 7-11 demonstrate the future of Phosphodiesterase 5 inhibitors, centrally acting drugs, local therapies, androgens and gene therapy in erectile dysfunction. Chapters 12 and 13 give perspectives about the future of targeting endothelium and Rho kinase for erectile dysfunction. Chapters 14-16 discuss the future of medical treatment of premature and retarded ejaculation. Chapters 17 and 18 detail the future of medical treatment of priapism and peyronie’s disease. Chapter 19 presents the medical treatment of femalesexual dysfunctions and the likely future aspects. And lastly chapter 20 gives perspectives about the pharmacogenomic aspects of sexual dysfunctions. Even as this text goes to press, however, we realize that newer drug discovery issues are being developed. The steady stream of new drug discovery issues will hopefully continue to justify more and more books on the treatment of sexual dysfunctions. Advances in Sexual Medicine: Drug Discovery Issues should be particularly useful for clinicians, basic scientists, and other researchers who are entering a new area of Sexual medicine research to experience the new challenges. It will serve as a valuable resource book for active researchers when they design new experiments. I am indebted to our authors who graciously accepted their assignments, and who have infused the text with their energetic contributions. I thank our patients who we hope will be the ultimate beneficiaries of this book. I am grateful to Prof. K-E Andersson and Prof. M A Ghoniem for writing the foreword.
