Migraine can significantly impact the quality of life. Several oral acute migraine treatments are available, and new specific drugs have been recently developed with the aim of limiting the impact of this disease on daily life. However, some patients do not respond to oral abortive drugs, presenting a challenge for neurologists and headache specialists. For this reason, neuromodulation and peripheral nerve blocks have emerged as a credible alternatives for abortive migraine treatment. Evidence from numerous randomized control trials (RCTs) and clinical studies has shown that these techniques effectively relieve migraine pain without causing consistent adverse events. Additionally, they offer several advantages, such as avoiding medication side effects and reducing drug-to-drug interactions. In this chapter, we will discuss the efficacy, tolerability, and safety profile of different neuromodulation techniques and peripheral nerve blocks for the acute treatment of migraine, focusing on RCTs and large real-life studies. We will also provide a description of their mechanisms of action, as well as their main contraindications.
