Recent technological advancements have introduced the exoscope as a potential substitute for operating microscopes in microlaryngeal surgery (MLS). Exoscope-assisted MLS allows for direct examination of the larynx in merely diagnostic procedures. Moreover, it allows for the surgical treatment of benign and malignant lesions. Proper magnification and visualization of the larynx are of utmost importance in this kind of surgery. The depth of focus, the ergonomics, and the 3D image quality provided by the VITOM 3D exoscope are well suited for MLS. This chapter aims to describe thoroughly the technical characteristics of the exoscope-assisted MLS, analyzing its feasibility and suitability. Finally, the authors' experience is presented and discussed to better define the advantages and disadvantages of this system.
