Simulation-based microsurgical training is usually performed in a laboratory setting with microscopes using live animal subjects and nonliving models. The refined movements and precision required in microsurgery make it technically challenging, and it is indeed associated with a very steep learning curve. We performed a preclinical study in a simulation laboratory setting using the VITOM 3D exoscope, demonstrating that exoscope-based simulation is absolutely feasible and a good option for future training programs. A comparison between the exoscope and the operating microscope was also performed in first-time users. An example of a preclinical application of the exoscope in microlaryngeal surgery is provided. Finally, the educational role of exoscopic technology in the operating room setting is also discussed, pointing out specific advantages and disadvantages of this system.
