Comments on. Improving Breast Cancer Surgery: A Classification and Quadrant per Quadrant Atlas for Oncoplastic Surgery (Clough KB, Kaufman GJ, et al. ASO DOI 10.1245/s10434-009-0792-y)

We like to comment the Clough’s article in which the authors attempt to do a classification of types of defects after breast cancer conservative treatment and respective breast oncoplastic reconstructive procedures. We debate about author’s indications regarding oncoplastic techniques used for the reconstruction of the upper pole of the breast, because in our opinion what they propose are not properly addressed to oncoplastic correction of these areas. Furthermore, we like to suggest authors an original procedure that we recently published on this journal, and in which we describe the use of a new type of quadrantectomy, that consist in a modification of the Wise-pattern technique for breast reduction, the procedure is indicated in all women with a tumour located in upper quadrants and with moderate to severe breast hypertrophy and/or ptosis type II. This procedure has the advantage of preserving the oncological benefit of the quadrantectomy without its well known aesthetic drawbacks,
